小兒及成人視網膜疾病

早產兒視網膜病變是嬰幼兒失明的主因,之前在台灣並無人專注在此領域研究,本人自2006年回國已為超過500個產兒施行雷射及眼內注射,並施行超過250台玻璃體切除手術。在關於早產兒視網膜病變的自然病史、流行病學、致病機轉、微創手術、抗新生血管物創新治療、視網膜OCT (Optical coherence tomography)、視力復健及長期解剖及功能成果及病人神經發育皆有論文創新發表。

在早產兒視網膜病變的自然病史,本人在台灣organize了一個prospective,multi-center早產兒視網膜病變自然病史的研究,台灣之前關於早產兒視網膜病變皆是回溯性的研究,我們發現我們的病人產生視網膜病變的時間比國外大型研究來的早,這是個很新的發現,我們已被知名的國際期刊RETINA接受。在早產兒視網膜病變的流行病學,我們首先發現媽媽的年齡是一危險因子,超過30歲的孕婦產生早產兒視網膜病變的小孩比小於30歲孕婦高,這篇文章刊行在眼科 Retina期刊上。此外,我們回顧分析出生體重超過1500 grams的早產兒視網膜病變病人,發現它的危險因子是房室內出血,而非低出生體重或周數,這篇文章已被Biomedical Journal刊出。

在微創手術方面,我首先將微創手術23-g vitrectomy運用於早產兒視網膜手術上,這篇研究也是刊行在眼科頂尖的Archives of ophthalmology上。在抗新生血管藥物治療早產兒視網膜病變方面,我組織了台灣的多中心臨床研究將台大、彰基、高長及本院的data整合,將用抗新生血管藥物(Avastin)治療早產兒視網膜病變的成果投稿到眼科臨床上可投稿最好的雜誌Ophthalmology,這篇論文也同時獲得國際外科學會佳作獎肯定。此外在抗新生血管藥物治療早產兒視網膜病變方面,我發現將用抗新生血管藥物(Avastin)玻璃體注射治療早產兒視網膜病變,會造成systemic serum VEGF受到抑制且抑制的時間長約2-3個月,比之前了解的抑制的時間約2個禮拜長許多,這篇文章被頂尖的期刊JAMA Ophthalmology接受。之後我們又比較了另一個抗新生血管藥物(Lucentis)與(Avastin)玻璃體注射治療早產兒視網膜病變造成systemic serum VEGF的程度,我們發現(Lucentis)對systemic serum VEGF抑制的程度不如(Avastin)厲害,約只有1周,這顯示(Lucentis)可能是較安全的藥,但還需要更多的研究證實。

我們也發現早產兒視網膜病變病人且有接受雷射或冷凍治療的病人的脈絡膜厚度比起沒有接受治療的病人厚度顯著要來的薄,顯示脈絡膜可能對早產兒視網膜病變的致病機轉有關,這個創新研究被接受刊行在眼科頂尖的期刊JAMA Ophthalmology上。另外在早產兒視網膜病變病人視力復健方面,我們與台大復健系蔡麗婷老師合作,發現這些小孩子接受視力復健之後反應時間可有效增快,且固視時間大幅增加,這篇文章也被頂尖的American Journal of Occupation Therapy接受。視力復健算是一個較新的領域,這些發現這些小孩經過適當的Training,也有再visual improvement的potential。

例外一個重要的領域是Long-term Outcome of Anti-VEGF for ROP (retinopathy of prematurity)過去病人在接受anti-VEGF bevacizumab的,病人的長期(約4-5歲時)macular structure及視力並未被報告過。這是一個prospective study,我們收入4到6歲曾經有因為type 1 ROP接受雷射或bevacizumab治療的小朋友,我們排除做過角膜或水晶體手術的病人。也排除了視網膜有皺摺或拉扯的病人。平均病人的年紀為5歲,我們主要測量的項目是病人的視力、optical components及macular structure。我們最主要的發現是這些接受anti-VEGF bevacizumab治療的和Laser或是Laser+IVB的病人比較起來,他們在5歲時有較好的未矯正視力、較深的眼前房深度及較薄的視網膜厚度及較高比例的Foveal depression,他們的choroidal thickness和雷射治療病人相較沒有顯著的差異。同時因他們也有較高的屈光異常,此異常主要是來自眼前節的異常而非眼軸長的增長所引起。我們這個研究也得到許多國際上的注意,我們也發現使用anti-VEGF治療的病人他們在結構上及功能上比較接近有早產但沒以治療的病人,這個論文已經在2018 American Journal of Ophthalmology所接受並發表。

最後是關於接受眼內注射抗新生血管藥物後病人的神經發育的結果。眼內注射抗新生血管藥物是治療早產兒視網膜病變一種創新性的治療,較傳統的雷射治療有以下的優點:①可有效回退早產兒視網膜病變②對視網膜血管破壞較小③對屈光不正有較佳的預後。先前我們的研究顯示抗新生血管藥物眼內注射後會進入全身的循環,造成全身的血管內皮細胞生長因子(vascular endothelium growth factor; VEGF)被抑制1週~3個月,由於血管內皮細胞生長因子是重要的神經發育生長因子,因此這些抗新生血管藥物雖然有效但有造成這些早產兒未來神經發育抑制的疑慮。因此本研究的目的是要了解過去有早產兒視網膜病變病人並接受抗新生血管藥物或雷射治療之早產兒病史小朋友之長期神經發育及相關的結果。我們此前瞻研究首先發現bevacizumab (VEGF 抗體)治療早產兒視網膜病變病人的3歲內的神經發育和有早產兒視網膜病變但不需治療的病人相似。這是第一個對bevacizumab治療早產兒視網膜病變的病患的神經發育及安全性提供前瞻證據的研究。因而此研究發表在眼科最頂尖的Ophthalmology期刊。

綜而言之,早產兒視網膜病變是一個終身的疾病,需要長期的追蹤,再經過適當的治療,多數的小孩可以有相當不錯的成果。相信在大家的努力下他們的未來是一片光明的。

四歲活潑可愛的程小弟回到眼科門診時,由於他在眼科經常回診,和眼科醫師及護理師很熟悉,他還跑過來給眼科醫師抱抱,除了身材略為瘦小並且帶著眼鏡,其實若不告訴你,你一定無法想像這個小朋友出生時體重不到900克,懷孕週期僅有27週,之前還曾因早產兒視網膜病變,雙眼需接受雷射治療。

早產兒視網膜病變是因早產,造成視網膜血管發育異常不完整,嚴重時會導致視網膜缺氧,血管內皮細胞生長因子 (vascular endothelium growth factor; VEGF)過度表達造成血管新生,而新生之血管拉牽視網膜造成視網膜剝離(retinal detachment),嚴重時會造成失明。

所幸目前在治療早產兒視網膜病變已有長足的近步。除了傳統的雷射治療,現在使用抗血管內皮細胞因子(Anti-VEGF)眼內注射,除了效果與傳統的雷射治療相當,也被發現有減少視網膜破壞,減少日後產生近視,散光,及斜視的優點。在雷射或眼內注射治療後,僅有少數的病人會繼續惡化到需要眼內手術,因而失明的機率已可大幅減少。

但這些過去有早產兒視網膜病變病史的病人和正常足月兒比較,眼睛的結構及功能上其實是有許多細微的差異。這些差別包過較多近視,散光,斜弱視的盛行率及較低視力等。在視網膜及脈絡膜的發育也有細微異常。長期來說,青光眼,白內障,視網膜破洞,視網膜剝離的發生率在這些病人也會較高。所以這些病人最好是能長期追蹤,以便若發現有異常時可以早期治療。

這些「慢飛天使」因先天發育較不完備,所以起始條件較差。這些小朋友需要多費心照料,家長不要給小朋友使用手機及平板,多鼓勵小朋友從事戶外活動及運動,以及注意營養。之後經過適切的治療及復健,這些病人也可以有近乎正常的視力以及神經發育。觀察他們從小到大,有些小朋友進步的幅度空間可真會令人大吃一驚呢!
